Appointments 

Rates vary based on the type of service and appointment that is provided. Please feel free to call or email for a list of rates and services.

Clear Perpective's services may be covered by your health insurance or benefits. Clear Perspective is happy to bill your insurance for you but deductibles and co-pays are your responsibilities and due at the time of service. You should know if prior approval is required by your insurance and if pre-approval is required from your primary physician. Payment by cash, check, Visa, MasterCard, American Express, or Discover is due at the time of your appointment.  Clear Perspective will bill insurance as long as you provide your insurance card within 7 days of the appointment.

Insurances currently accepted are:

  • Blue Cross Blue Shield

  • Medicaid

  • Aetna

  • Cigna

  • MedCost

  • Medicare

  • Magellan 

  • MultiPlan

 

 

Due to the complex needs of our clients and our employees, only certified service dogs will be allowed in the office.   Some of our clients may have allergies to animals or specific phobias that do not make encountering an animal a healthy experience.  Because we respect the rights of all of our clients we kindly request that emotional support animals are left home during the session.

Clear Perspective's cancellation policy is that a 24 hour notice is required to cancel your appointment. If you do not give advance notice of cancellation, Clear Perspective may charge you $50 for your booked session.
